Before Training Starts

Induction information guidance 2011 - guidance on what Induction Sessions GP Trainees should attend

Form R - Form R to be completed by all trainees on joining a GP training programme so that the Deanery can issue their National Training Number. Trainees must complete a Form R for each year of their training.

Conditions of joining a GP training programme. - Trainees must read and sign the conditions of training document at the start of their GP training programme.

Educational Agreement between Deanery and GPStR 2011 - The GP Trainee should send a signed copy of this form when they accept a GP Training Programme in the Wessex Deanery 

Educational Agreement between Trainer and GPStR 2011 - Document setting the commitments and responsibilities of the Trainee and the Trainer (Not to be sent to the Deanery).   

GP Trainee Contract of Employment - GSTR Framework for a written contract of employment – for ST1 and ST2 trainees during their GP placement and for all GP ST3 trainees  (A new contract is needed for the ST3/GPStR post even if staying in the same practice)

PAY1 form 2012 - Pay authorisation form for: General Practice Registrar Payment for each ST1/2 GPR and ST3/GPStR post.

PAY2 form 2011 - Pay authorisation form for: Change of GP Registrar Details / Circumstances.  Completion of this form is required to enable the Deanery to adjust their records and continue to release payments (such as GP registrar salary, GP trainer’s grant) to the training practice.

PAY1-TG form 2011 - Authorisation form for payment of a Trainer's Grant only. Eg for doctors with external funding accepted for a period of general practice training (eg Hospital specialist, doctor not eligible for funding, Defence Deanery trainee)
